You may not create an account or submit an application on behalf of another person. This includes employers, licensing companies,
and third party services.
Each individual is responsible for their own account, login credentials, and all information submitted including legal attestations and
background check authorization.
If you need to manage a facility permit, you must first create your own individual account then request administrative access from
the designated representative or supervising pharmacist.
All users must have their own account, even if they are not applying for a license or registration. Shared or third-party accounts
are not permitted.
Personal license numbers should be entered with applicable prefix/suffix.
If you do not know your license number or license type, you may attempt to locate it via
license verification.
If you are registering as an unlicensed administrator of a pharmacy or facility, please enter that permit number and type and the associated supervising pharmacist or
designated representative will be notified to approve your request.
NOTE: If you administer multiple pharmacies and/or facilities, it is the responsibility of each Pharmacy Supervising Pharmacist or Facility Designated Representative to add you as an
approved administrator. You only need to create one profile. This profile can be linked to multiple pharmacies/facilities as an administrator.
